For new patients and patients that have not been here in a while and need to update paperwork, please arrive approximately 10 minutes early to fill out some paperwork for us. We will also need to see your insurance card if we have not seen you before, or if you have updated/changed your insurance since the last time you last visited.

If you would like to request patient history reports for legal cases, other health care professionals, or other reasons, please contact us and we will be happy to work with you and put together a report for you. We will need your consent, so please call us for more information, if you would like to look into requesting your patient history report.

If you have had any changes in your health (i.e., recent cancer diagnosis, heart issues/stints placed, etc.), please notify us before Dr. Miller works with you, as we may need to change your health plan.
